Capture the record at the point it happens
A record reconstructed on Friday afternoon is a memory test, and memory loses disputes. Design every capture so it happens at the moment of the event, by the person it happens to: the instruction logged on the phone that took the call, the delivery photographed at the gate, the verbal agreement written down before the meeting breaks up. If capturing something takes more than a minute, it will stop happening in the first busy week, so keep shrinking the step until it survives one.
