Acceleration usually starts as a conversation: get it back on programme, we'll sort the money. Extra gangs go in, weekend working starts, the costs run from day one - and the instruction that triggered it all exists nowhere. When the account lands, the same people who asked for the push are asked to pay for it, and the absence of paper becomes their best argument: we never instructed that.

Acceleration is exactly the kind of change that must be confirmed before the cost runs, because the spend is deliberate, large and visible: what was asked, by whom, what resource it means, what it is expected to cost. The record template is that confirmation in usable form - and if the instruction is disputed later, the contemporaneous record is the difference between a claim and an anecdote.
