What's happening?

A field stops being filled in, a feed breaks, a formula gets overwritten by an innocent edit. Nothing announces any of it. A register that has been blank for three weeks looks exactly like three quiet weeks, and the first anyone learns otherwise is a wrong number in a meeting that mattered.
